I believe the answer is: a natural entity versus personal experience.
Disease tend to caused by wide spread epidemic that cause a lot of people within a region to have abnormal body function. (such as virus epidemic)
Illness on the other hand, is the abnormal body function that happen because our way of living (such as diet, or personal hygiene)
